Provide Consent >

a) You will be asked to agree with our terms and conditions, including our fees, and penalties for late cancellations and no-shows. b) You will be asked to provide consent for your information to be shared with other Kintsugi team members, to allow for a comprehensive multi-disciplinary approach to your management plan. We will always check with you, before including your information in a case discussion. c) You will also be asked for permission for your de-identified data to be used for programme evaluation purposes.

Personalised Treatment Plan >

The next step is to develop a treatment plan that takes into account your health priorities. Your plan will typically address all four lifestyle pillars, tailored to specific issues identified during your initial assessment. It will include clear goals and objectives, so you can track your progress. We know that there will be ongoing challenges, and that it may be a matter of 2 steps forward, one step back. We are happy to adjust expectations, or even to go back to the drawing board and try a new strategy, if necessary. Our focus is on long-term sustainability, rather than short-term gain. Any step forward is a win, and we accept that sometimes it takes as long as it takes.

Agreement >

We want you to own your treatment plan. You are responsible for implementing it, we are responsible for helping you implement it. You need to agree with the goals and objectives, and be committed to giving it your best go. We are happy to tweak your plan until you feel comfortable with it.

Implementation >

Our commitment is to provide you with the tools you need to implement your plan. We consider education as one of the most important tools in your toolbox. You will have access to online educational presentations, as well as workbooks. The purpose of the workbook is to help you apply what you learn to your personal circumstances, to set your own goals, and to identify potential challenges and think about ways to overcome them. The face-to-face educational sessions are available to help clarify and expand on the reading you’ve done at home. Review appointments are available as either individual consultations (face-to-face or telehealth) or as part of our Living Room sessions.

Ongoing Support and Motivation >

Inspiration and motivation can wax and wane, and some weeks may be more difficult than others. You will be supported on your journey to better health with weekly contact from us in the form of educational emails and encouraging SMS messages. You will also have access to a 2 weekly phone call with one of our nurses to check in and ask questions and hopefully be inspired to keep going.

Initial Assessment >

Your initial assessment includes a comprehensive health and lifestyle questionnaire, preferably completed prior to your first appointment. We gather as much information as possible before developing a timeline and matrix of your symptoms in the context of lifestyle and significant events. This will be followed by a face-to-face consultation to confirm and expand on the information you provided, so that we can pull it together into a story that accurately reflects your health journey. You may be referred for further tests where appropriate. Some of these may be functional tests not covered by Medicare.
